The CHU and the University of Bordeaux have joined forces to offer a simple and effective solution to help patients in Nouvelle-Aquitaine in the management of their disease: Rafael This application is part of the Guardian Angel program carried by these same actors and aims to:- Help you as a patient in the daily management of your disease through validated and reliable documentation- Share follow-up information with your doctor, who is also a user of the application, to help him adapt his treatment as closely as possible to your situation- Record all the events of your life as a patient that impact your health via a personal diary or questionnaires Beyond the Rafael application, a whole organization is set up to help you, when you need it:- Your healthcare team, which has access to more complete information about your situation- A Local Coordination Support System piloted with the Bordeaux University Hospital which can intervene at your request or that of a member of your care team to support you when you need it- Experts from Bordeaux University Hospital who can be mobilized by your healthcare team The currently supported diseases are rheumatoid arthritis, asthma, diabetes and COPD.With Guardian Angel and its Rafael application, you are no longer alone in the face of your illness!
